I would like to register for the following Commando Krav Maga Intensive Boot Camp:
Please read the following conditions: TERMS & CONDITIONS A. The Commando Krav Maga (CKM) Intensive Boot Camp is to be taught by the Founder and Chief Instructor, Moni Aizik, or an authorized and qualified CKM Instructor assigned specifically by Moni Aizik. B. I, the applicant, have no criminal record and authorize Combat Survival Inc to conduct a criminal background check before I am accepted. C. I am physically and mentally capable to take the CKM Intensive Boot Camp course. D. I am participating in the Commando Krav Maga Intensive Boot Camp for self-improvement purposes only and understand that I will not be required to fulfill the testing components for CKM Instructors as I am not going to teach Commando Krav Maga. E. However, if I decide that I want to become a CKM Instructor during my course, I have the option to take the CKM Instructor Final Exam. Upon successful completion of the Exam and by agreeing to the terms and conditions for CKM Instructors, I will then have the right to enjoy the benefits of becoming a certified CKM Instructor for the appropriate level. F. I understand that if my application is approved, all course fees of US$1,955.00 must be paid prior to the course start date (up to 3 installments if necessary) and are non-refundable. G. In the event that I am unable to attend or complete the Commando Krav Maga (CKM) Intensive Boot Camp that I am registered in, I will be allowed to reschedule to another CKM Intensive Boot Camp, free of charge, for the same level within a 1-year period. H. My cost for progressive CKM Intensive Boot Camps will always remain the same as my original course fee, regardless of future fee increases. I. Upon completion of the CKM Intensive Boot Camp, I will receive the official Commando Krav Maga curriculum for the Level(s) for which I am participating and a prestigious CKM Intensive Boot Camp Certificate of Achievement.
