Please read the following conditions set forth: 1. TERMS & CONDITIONS A. The Commando Krav Maga (CKM) Instructor Certification Course is to be taught by the Founder and Chief Instructor, Moni Aizik, or an authorized and qualified CKM Instructor assigned specifically by Moni Aizik. B. I, the applicant, have no criminal record and authorize CKM International headquarters or Combat Survival Inc to conduct a criminal background check before I am accepted. C. I am physically and mentally capable to take the CKM Instructor Certification course. D. Upon successful completion of the course, which includes passing the Instructor Certification Exam, I will be certified as a CKM Instructor for the appropriate Level. E. Upon certification, I will be given a prestigious CKM Instructor Certification Diploma, which also acts as my teaching license, along with all the benefits exclusive to certified CKM Instructors. F. Combat Survival Inc will not charge me any licensing fees as a certified CKM Instructor. However, for quality control reasons, I will need to progress to the next level within a 1-year period after my certification date to keep my CKM teaching license current and valid. My cost for future progressive CKM Instructor’s courses (next level and up) will never increase from my original course fee, regardless of future fee increases. G. As a Certified CKM Instructor, I will be allowed to teach CKM students (men and women 14 and up) up to and including the level(s) for which I am certified. H. I understand that if my application is approved, all course fees of US$1,955.00 must be paid prior to the course start date (up to 3 installments if necessary) and are non-refundable. I. In the event that I am unable to attend or if I do not pass the Commando Krav Maga (CKM) Instructor Certification Course that I am registered in, I will be allowed to reschedule to another CKM course, free of charge, for the same level within a 1-year period. J. As a certified CKM Instructor, I will be entitled to attend, free of charge, any National CKM workshops designated specifically for CKM Instructors. This includes training in any of the CKM Intensive Boot Camps for my level. 2.
